AFFIRMATIVE FORM: The affirmative form to the verb to be is a simple sentence in present tense look at the example

She is a excellent mother
answers: affirmative form: yes she is a excellent mother.
negative form: no, she is not a excellent mother.
interrogative form: is she a excellent mother?
THE NEGATIVE FORM:
To a negative form sentence wer place the negative Word No and NOT in the sentence.
example:

answers: affirmative form: yes he is a teacher.
interrogative form : is he a teacher?
THE INTERROGATIVE FORM
To pass a sentence to the interrogative form we translate the verb to be at the beginning of the sentence then copy the rest equal and end the sentence with the question mark.
